Catastrophic Health Insurance Plans: Are They Right for You?

Catastrophic health insurance plans offer the lowest premiums on the ACA Marketplace, but they come with significant trade-offs. Designed to provide a safety net against worst-case medical scenarios, these plans are not for everyone. Understanding who qualifies, what they cover, and when they make financial sense can help you decide if a catastrophic plan is the right choice.

Who Qualifies for Catastrophic Plans?

Catastrophic health insurance plans are available through the ACA Marketplace to two groups of people:

  • Individuals under age 30: If you are under 30 at the time of enrollment, you are automatically eligible to purchase a catastrophic plan.
  • Individuals with a hardship or affordability exemption: If you are 30 or older and can demonstrate that other Marketplace plans are unaffordable (costing more than a certain percentage of your income) or that you have experienced a qualifying hardship, you may obtain an exemption that allows you to enroll in a catastrophic plan.

It is important to note that catastrophic plans are only available as individual coverage — they cannot be purchased as family plans. Additionally, premium tax credits and cost-sharing reductions cannot be applied to catastrophic plans, so you will pay the full premium out of pocket.

What Catastrophic Plans Cover

Despite their high deductibles, catastrophic plans still cover all essential health benefits required by the ACA once you meet your deductible. Before the deductible is met, catastrophic plans cover three primary care visits per year and certain preventive services at no cost. This means you can still receive annual wellness exams, vaccinations, and recommended screenings without paying out of pocket.

The key numbers to understand are the deductible and the out-of-pocket maximum. For 2026, catastrophic plan deductibles are typically set at the ACA out-of-pocket maximum limit, meaning you will pay for nearly all of your medical care until you hit that threshold. After that, the plan covers 100% of covered services for the rest of the plan year.

When a Catastrophic Plan Makes Sense

A catastrophic plan may be a smart choice if you are young, healthy, rarely need medical care beyond preventive services, and want the lowest possible monthly premium. It serves as true insurance — protecting you from devastating financial consequences if you have a serious accident or unexpected illness. However, if you take regular medications, manage a chronic condition, or anticipate needing medical services throughout the year, a Bronze or Silver plan with subsidies may actually cost you less overall.
